Obtaining a copy of your collision report is easy. Simply send a check or money order in the amount of $5.00, payable to the Union County Sheriff's Office, to the address below. Please put the report/local code number provided by the Sheriff or Deputy on your check or money order, along with the date of the collision.

Pursuant to KRS 189.635, only involved parties are allowed to receive a copy of the collision report. Involved parties include persons involved in the collision/accident, registered vehicle owners, the parents or guardians of a minor who is a party to the collision/accident, insurers or their written designees for insurance business purposes of any party who is the subject of the report, or to the attorneys of the involved parties.

File a Civilian Traffic Collision Report with the Kentucky State Police

Pursuant to KRS 189.635(4): Any person operating a vehicle on the highways of the state who is involved in an accident resulting in any property damage exceeding five hundred dollars ($500) in which an investigation is not conducted by a law enforcement officer shall file a written report of the accident with the Department of Kentucky State Police within ten (10) days of occurrence of the accident upon forms provided by the department.
