Family Caregiver Support Program

Assistance is provided for individuals who are caring for a person age 60 or older; or for a relative, age 55 or older, who is caring for a grandchild under age 18. The goal of the program is to help caregivers improve their health and well-being by providing information, training, and assistance they need. Home Delivered Meals

A hot, nutritious lunch meal is provided Monday through Friday for individuals over the age of sixty (60) who are impaired in some way that makes them unable to cook for themselves; there is no one in the home to cook for them; they do not drive and they are unable to attend a senior center to eat a meal. This service is commonly referred to as “Meals on Wheels.”  The program is based on a priority of need score.  If you feel someone meets these qualifications, please call 800-928-9094 to make a referral.

Housekeeping or Homemaker Services

Provides assistance with light housekeeping, laundry, shopping, and meal preparation for individuals who are over the age of sixty (60) who are impaired in some way that makes them unable to perform tasks for themselves and there is no one in the home to do those things for them. If you feel someone meets these qualifications, please call 800-928-9094 to make a referral.

National Council on Aging (NCOA)

GRADD’s Benefits Enrollment Center helps low-income seniors and persons with disabilities find and enroll in all the benefits programs for which they are eligible, and create coordinated, community-wide systems of benefits access.  For more information, contact Sarah Ackerman at 800-928-9094 or (270) 852-1321.

Personal Care Services

Provides assistance with bathing, dressing, toileting, and other personal needs for individuals who are over the age of sixty (60) who are impaired in some way that makes them unable to perform tasks for themselves and there is no one in the home to do those things for them.  If you feel someone meets these qualifications, please call 800-928-9094 to make a referral.

Respite

Provides caregiver relief by offering supervision and care for an impaired person, over the age of sixty (60), in their home.  If you feel someone meets these qualifications, please call 800-928-9094 to make a referral.

Senior Medicare Patrol

GRADD’s Senior Medicare Patrol (SMP) program is seeking to recruit volunteers in Daviess, Hancock, Henderson, McLean, Ohio, Union, and Webster counties to help Medicare beneficiaries avoid, detect, and prevent health care fraud.  For more information, contact Kelli King, SMP Coordinator at (270) 926-4433 or (270) 684-2823.

State Health Insurance Assistance Program (SHIP)

GRADD’s State Health Insurance Assistance Program (SHIP) provides one-on-one assistance with Medicare, Medicaid, and other public and private insurance.  Locally certified SHIP counselor, Melissa Polites, can assist you with Medicare Part D (prescription) plan choices as well as accessing resources for medications that are not covered.  We also assist with Medicare Part D comparisons during the annual open enrollment, which is October 15 to December 7.  For the 2022 Open Enrollment period, we saved Union County residents $79,012.75 on premiums and co-pays.  Call the Senior Services Office at (270) 389-4550 for more information.

Veteran Directed Care

Veteran Directed Home and Community Based Services gives Veterans of all ages the opportunity to receive the Home and Community Based Services they need in a consumer-directed way.   For more information,  please call 800-928-9094.
